Multiple Choice Forms:
This is an example of a multiple choice form. More than one item can be
selected by clicking on the box at the beginning of the line.

Forced Choice Forms:
These are examples of forced choice forms.
The first involves toggle buttons. Only one item can be selected at
a time.
A second option involves an option menu. One item can be selected by
holding the mouse button down on the option in the rectangle. All of the
choices are displayed. Selection is made by positioning the cursor on one
of the options and releasing the mouse button. The selection is then displayed.

Feedback forms accept open text with virtually no limitation on the number
of words.
